- We encourage engagement through our Enhanced Services program. By documenting completed clinical services for available patients, you have an opportunity to earn revenue while providing essential care. Be sure to log in to EQUIPP regularly to review how Enhanced Services can benefit your pharmacy.
- It’s crucial to prioritize your patients and ensure their adherence to medication. Printing your patients’ page weekly and tracking their adherence in each PDC measure can positively impact their outcomes. By reviewing outliers and grouping patients appropriately, you can better identify areas for improvement.
- Monitoring your performance data is integral to driving success. The main screen of your pharmacy dashboard features monthly score trends in each quality measure, such as PDC and others. The visualizations provide valuable insights into how your pharmacy is doing, allowing you to make informed decisions.
- When evaluating specific measures, selecting the name of the measure will take you to a detailed drill down. Here, you can access benchmarking data and compare your performance to others. These insights will help you identify areas of strength and opportunities for growth.
- To track your progress towards a specific health plan’s goal, be sure to regularly check the Quality Improvement Program (QIP) table. This will allow you to evaluate your population’s performance and identify patient-level performance improvement opportunities so you can meet the set goal.
