Get a Jump on Medication Adherence with EQUIPP’s Year-to-Date Data

EQUIPP year-to-date (YTD) data for 2024 is available with the May release, and your pharmacy staff is now able to review performance for months, January through March. Your team will be able to track progress for specific quality measures and understand performance at the measure level, program level and most importantly, be able to review your patients’ adherence.

Focusing on the Patients

With January – March data, pharmacies can get ahead of the adherence decline that happens over the course of the year and focus on the patients.

Act now, act early, and use the YTD data to understand the who, where, and when of medication adherence.

  • Identify those patients that are close to the 80% threshold and work to get those patients over the line
  • Keep patients above the 80% threshold because as the year progresses, time becomes a larger barrier to achieving adherence

Quickly Access Patients

The All Patients View in EQUIPP displays each patient attributed to at least one quality measure in your pharmacy. It’s the quickest way to see all your patients; just click the person icon on the navigation bar on the left side of the screen. Review these patients to monitor their progress on all measures from now through year-end.

Measure Level Review

Users can create action plans to improve pharmacy performance based on the data in EQUIPP. By focusing on the YTD view and then selecting a quality measure, you can access the measure performance screen. This page allows you to review performance score goals, address measure gaps, and prioritize payer programs in the Quality Improvement Program (QIP) table.

Review Your QIP Table checklist:

  • Sort programs by highest number of patients to lowest
  • Set internal plans to achieve the program goal of a health plan at minimum
  • Keep track of the ‘goal met’ indicator through the year

Program Level Review

It’s important to check the My Programs and Enhanced Services tabs, as payer programs are activating patient opportunities for pharmacies throughout the course of the year. These programs offer incentives for pharmacies to address gaps in care at the patient level by providing primary care services.

Here are some tips for getting the most out of EQUIPP:

    • Log in to the EQUIPP dashboard weekly.
    • Create a quality champion to oversee EQUIPP tasks.
    • Utilize staff to complete tasks such as blood pressure readings, scheduling, refill reminders and more.
    • Understand your goals and how to influence your patients.
    • Be bold and try new things to improve performance.
